Misunderstanding the Sacraments

Sometimes I think we grossly misunderstand the sacraments of baptism and communion when we just make them a part of our liturgy.

I wonder if, when Jesus said, "baptize them in the name of the Father, the Son, and the Spirit," he meant more than simply getting people wet? Perhaps he meant for us to immerse people into a Trinitarian reality. That would mean becoming a Trinitarian community in which we could immerse disciples.
And what of communion? Was the "remembering" relegated to the time at the end of service, or was he instructing us to become the type of community who, by our very lives, live out a constant anamnesis?
